Job summary

Heart’n Soul Hospice is seeking a qualified RN Case Manager in Seattle, WA to lead comprehensive assessments and coordinate care for patients with life-limiting illness. You will collaborate with physicians and a multidisciplinary team to implement and adjust care plans, ensuring compassionate, high-quality end-of-life care.

The role requires an RN license, driving capability, and reliable transportation. Two years of RN experience is preferred, with a commitment to empathy and patient-family

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical settings.

Responsibilities

• Initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborates with patients, families, physicians and other healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with the team.
• Perform on-call responsibilities and provide on-call service as assigned.
• Timely submission of documentation and compliance with laws and policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

I was recently accepted to the American Academy of Hospice and Palliative Medicine’s (AAHPM) Next Gen Scholars Program as one of 8 scholars comprising medical residents, fellows, and post-graduate students across the country.I’ll link more information to what the program entails below, but I am yet grateful for the opportunities and exposure I am gaining through Heart’n Soul Hospice that I believe is orchestrating my future endeavors and career path.
