Job summary

AMG Specialty Hospital – Northwest Indiana is seeking a Clinical Liaison to support transitions for medically complex patients in the South Bend area. Jurisdiction includes travel to hospitals, referral sources, and partner meetings.

This full-time role requires clinical credentials (RN/RT/LPN/PT) and a proven relationship-focused approach to educate providers, grow referrals, and improve admission processes for AMG's post-acute hospital services.

Qualifications

• RN, RT, LPN or PT licensure required.
• Full-time Clinical Liaison role with clinical expertise and relationship development.

Responsibilities

• Evaluate referred patients for clinical appropriateness.
• Educate physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services.
• Develop and maintain strong relationships with referral partners.
• Identify patients who may benefit from long-term acute care hospital services.
• Support timely communication and a smooth referral and admission process.
• Help increase appropriate referrals, admissions, and hospital census.
• Manage and grow an assigned territory.
