Job summary

AMG Specialty Hospital – Northwest Indiana seeks a Clinical Liaison to connect Chicago-area hospitals with our specialized care. The role requires daily travel to hospitals, referral sources, and partners, to transition patients to AMG for continued hospital-level care.

The ideal candidate holds an active RN, RT, LPN, or PT license, brings hands-on clinical experience, and excels at building relationships with physicians and discharge planners.

Responsibilities

• Support timely communication and a smooth referral and admission process.

About the Opportunity

We are seeking a motivated and relationship-focused full-time ClinicalLiaison who is a Registered Nurse (RN) , Respiratory Therapist (RT) , LicensedPractical Nurse (LPN) or Physical Therapist (PT).

This role combines clinical expertise, relationship development, andpatient advocacy. The Clinical Liaison helps appropriate patients transition toAMG for continued specialized hospital-level care.

The position primarily covers Chicago area hospitals and requiresdaily travel to hospitals, referral sources, meetings, and prospective referralpartners.
