Job summary

AMG Specialty Hospital – Northwest Indiana is seeking a Clinical Liaison (RN/RT/LPN/PT) to facilitate transitions to AMG for ongoing hospital-level care. The position covers South Bend area hospitals with daily travel to referral sources and partners.

You will evaluate referrals, educate providers about AMG services, and build strong relationships to grow referrals and census in the Northwest Indiana territory.

Qualifications

• Must hold RN, RT, LPN, or PT license.
• Willingness to travel daily to hospitals within the territory.
• Ability to educate physicians, case managers, discharge planners and other healthcare professionals.

• Develop and maintain strong relationships with referral partners.

• Manage and grow an assigned territory.

About the Opportunity

We are seeking a motivated and relationship-focused full-time ClinicalLiaison who is a Registered Nurse (RN) , Respiratory Therapist (RT) , LicensedPractical Nurse (LPN) or Physical Therapist (PT).

This role combines clinical expertise, relationship development, andpatient advocacy. The Clinical Liaison helps appropriate patients transition toAMG for continued specialized hospital-level care.

The position primarily covers South Bend area hospitals andrequires daily travel to hospitals, referral sources, meetings, and prospectivereferral partners.
