Job summary

AMG Specialty Hospital – Northwest Indiana seeks a Clinical Liaison to connect patients with specialized hospital-level care. The role centers on clinical expertise, relationship-building with physicians, discharge planners, and referral partners, and advocacy for medically complex patients.

You will travel daily across Chicago-area hospitals, referral sources, and partner sites to evaluate referrals, educate stakeholders, and ensure smooth transition and admissions.

Qualifications

• Active licensure as RN, RT, LPN or PT is required.
• Hands-on clinical experience is preferred.
• Strong communication and relationship-building skills.

Responsibilities

• Evaluate patients for clinical appropriateness for AMG services.
• Educate physicians, discharge planners and other health professionals about AMG services.
• Develop and maintain relationships with referral partners.
• Coordinate with hospitals and referral sources for timely referrals.
• Travel daily within the Chicago area to hospitals and partners.

About the Opportunity

We are seeking a motivated and relationship-focused full-time ClinicalLiaison who is a Registered Nurse (RN) , Respiratory Therapist (RT) , LicensedPractical Nurse (LPN) or Physical Therapist (PT).

This role combines clinical expertise, relationship development, andpatient advocacy. The Clinical Liaison helps appropriate patients transition toAMG for continued specialized hospital-level care.

The position primarily covers Chicago area hospitals and requiresdaily travel to hospitals, referral sources, meetings, and prospective referralpartners.

What You’ll Do

• Evaluate referred patients forclinical appropriateness
• Educate physicians, casemanagers, discharge planners, and other healthcare professionals aboutAMG’s services
• Develop and maintain strongrelationships with referral partners
• Identify patients who may benefitfrom long-term acute care hospital services
• Support timely communication anda smooth referral and admission process
• Help increase appropriatereferrals, admissions, and hospital census
• Manage and grow an assignedterritory

• Previous hands-on clinicalexperience
