Job summary

AMG Specialty Hospital – Northwest Indiana seeks a Clinical Liaison to connect patients with specialized hospital-level care. The role centers on clinical expertise, relationship-building with physicians, discharge planners, and referral partners, and advocacy for medically complex patients.

You will travel daily across Chicago-area hospitals, referral sources, and partner sites to evaluate referrals, educate stakeholders, and ensure smooth transition and admissions.

Qualifications

• Active licensure as RN, RT, LPN or PT is required.
• Hands-on clinical experience is preferred.
• Strong communication and relationship-building skills.

Responsibilities

• Evaluate patients for clinical appropriateness for AMG services.
• Educate physicians, discharge planners and other health professionals about AMG services.
• Develop and maintain relationships with referral partners.
• Coordinate with hospitals and referral sources for timely referrals.
• Travel daily within the Chicago area to hospitals and partners.
