Job summary

Qualifications

• Active licensure as RN, RT, LPN or PT listed above.
• Hands-on clinical experience in healthcare settings.
• Proven ability to communicate effectively with physicians and care teams.

Responsibilities

• Evaluate referred patients for clinical appropriateness.
• Educate physicians, case managers, discharge planners and healthcare professionals about AMG’s services.
• Develop and maintain strong relationships with referral partners.
• Identify patients who may benefit from long-term acute care hospital services.
• Support timely communication and a smooth referral and admission process.
• Help increase appropriate referrals, admissions and hospital census.
• Manage and grow an assigned territory.
