Job summary

Acadiana Management Group is seeking a Clinical Liaison for an on-site role in Muncie, IN, with territory near Indianapolis, to coordinate patient referrals and admissions to post-acute LTAC services.

Qualifications

Role Description

The Clinical Liaison is a full-time, on-site role based in Muncie IN with territory near Indianapolis, responsible for coordinating patient referrals and admissions to Acadiana Management Group’s post-acute LTAC services. In this role, the Clinical Liaison evaluates clinical information to determine appropriate levels of care, collaborates with physicians, hospital staff, and community providers, and communicates clinical capabilities and service offerings to referral sources. Day-to-day responsibilities include conducting patient assessments, explaining treatment options and facility services to patients and families, and facilitating smooth transitions from acute care or home settings to the organization’s facilities. The Clinical Liaison also supports outreach and relationship-building with healthcare partners, participates in care planning discussions, and maintains accurate documentation to ensure continuity of care and compliance with organizational standards.

Qualifications

• Clinical coordination experience, including Case Management skills to assess needs, plan transitions, and support appropriate post-acute care placement.
• Background in Home Care or community-based services, with the ability to collaborate with home health providers and understand continuity of care across settings.
• Sales and Business Development skills to build strong relationships with referral sources, communicate service value, and support growth objectives.
• Knowledge of long-term acute care patient populations.
• Active clinical licensure (e.g., RN, PT, LPN, RT or other relevant credential) preferred, with experience in acute or post-acute care settings.
• Strong interpersonal, communication, and presentation skills, with the ability to work collaboratively with multidisciplinary teams and engage patients and families.
• Proficiency in electronic health records and basic office software, with strong organizational skills and attention to detail.
• Bachelor’s degree in a healthcare-related field or equivalent clinical experience; previous experience in a liaison, admissions, or outreach role is highly beneficial.
