Meadow Park Rehabilitation and Nursing Center is seeking a Clinical Liaison to strengthen connections among hospitals, partner organizations, and our facility. You will support smooth transitions, enhance referrals, and uphold our care standards across settings.
The role requires a valid driver’s license for facility travel, RN/LPN/LSW licensure preferred, and an associate’s degree with a bachelor’s preferred. Excellent communication and teamwork are essential.
