Job summary

Marquis Health Consulting Services is seeking a Clinical Liaison to strengthen clinical relationships and coordinate care across hospitals and partner facilities.

The role emphasizes communication, integrity, and seamless transitions, with responsibilities including meetings, referrals, and end-of-day reports. Travel between facilities is required; RN/LPN/LSW licensure is preferred; associate degree is required, bachelor’s degree preferred.

• Ability to work collaboratively with residents, care teams.
• Strong organizational skills with attention to detail and accuracy in documentation.

Responsibilities

• Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
• Promote the organization’s philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
• Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
• Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
• Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
• Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
• Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
