Vynca Inc. is hiring a Clinical Intake Specialist to work with the Enhanced Care Management team. You will engage clients by phone, review charts, and build person-centered care plans with goals of care. The role emphasizes rapid intake, documentation, and collaboration with the care team.

Candidates should have an LVN license in California or a related degree/certification, 2+ years in intake, and strong communication skills. Remote/in-person work options available; Pacific Time hours apply.

Qualifikationen

• Active, unrestricted California LVN license, or a Bachelor's degree in a related field, or a Medical Assistant certificate.
• Minimum of 2 years intake/admissions experience in a high-volume environment.
• Experience with complex care ECM populations.
• Experience with Health Gorilla or Health Record Systems.
• Strong oral and written communication skills.
• Bilingual English/Spanish highly preferred.
• Ability to work Monday-Friday 8:30am-5pm Pacific Time, with potential evenings/weekends.

Aufgaben

• Conduct outreach to newly referred clients and log activity in VyncaOS.
• Conduct 8-12 intake visits daily with efficient assessments.
• Assist clients in completing initial intake visits including psychosocial assessment and care plans.
• Review medical charts and document client medical needs and interventions.
• Communicate critical information to the care team for personalized planning.
• Utilize evidence-based practices such as Motivational Interviewing and Trauma-Informed Care.
• Attend weekly Team Operations Meetings.
• Maintain a structured schedule to optimize intake and engagement.
• Provide warm transfers from field teams and ensure same-day responses.
• Thrive in a fast-paced environment with strong KPI focus.
