Job summary

Residential Home Health and Hospice is seeking a Hospice Clinical Field Staff Supervisor in Troy, MI. This role leads field staff, ensures adherence to case management guidelines, and coordinates patient assignments within a multidisciplinary team to deliver high-quality home-based hospice care.

The position requires an RN or BSN with a current state license, at least 2 years in hospice, and 1 year of supervisory experience.

Qualifications

• Licensed RN or BSN with current state license.
• Minimum of 2 years of Hospice experience.
• Minimum 1 year of supervisor experience.
• Familiarity with all regulations pertaining to Hospice operation.

Responsibilities

• Ensure productivity goals of clinical staff are met.
• Ensure case management guidelines are followed by each team member.
• Assign patients to multidisciplinary team and adjust schedules as needed.
• Serve as the first line for resolution of concerns and issues for the staff.
• Train, develop, and mentor staff, including orientation, patient assignment, and employee reviews.
• Communicate with support staff to assess needs and assign patients.
• Engage leadership on continuous quality improvement and problem solving.
• Develop staff to meet quality, safety, and competency requirements.
