Job summary

Parkview Health in Fort Wayne, IN seeks a Registered Dietitian for PACE to join onsite nutrition care. You will perform nutrition assessments, determine diagnoses, implement interventions, and monitor progress as part of the multidisciplinary team.

You will educate patients and staff, serve as a preceptor for dietetic interns, and help develop policies. Requires Bachelor's in Dietetics; Indiana license within 6 months; Master’s preferred and ACEND internship completed.

Qualifications

• Knowledge of Nutrition Care Process and documentation.
• Ability to counsel and motivate patients.
• Experience with nutrition support services.
• Strong communication with multidisciplinary teams.

Responsibilities

• Perform nutrition assessments and diagnose nutrition problems.
• Develop and monitor evidence-based nutrition interventions.
• Communicate nutrition care plan with the care team.
• Educate patients and families on nutrition recommendations.
• Serve as a preceptor for dietetic interns.
