Parkview Health in Fort Wayne, IN seeks a Registered Dietitian for PACE to join onsite nutrition care. You will perform nutrition assessments, determine diagnoses, implement interventions, and monitor progress as part of the multidisciplinary team.

You will educate patients and staff, serve as a preceptor for dietetic interns, and help develop policies. Requires Bachelor's in Dietetics; Indiana license within 6 months; Master’s preferred and ACEND internship completed.

Job description

Registered Dietitian for PACE
  • The Registered Dietitian (RD) will support PACE (program for all-inclusive care of the elderly) patients. The RD will work onsite alongside other healthcare providers for nutrition-related problems for a variety of diagnoses, including chronic disease management and management of nutrition support.
  • This is an onsite position to support PACE, Hospital to Home, Home Health, and acute inpatient care
  • The RD is an integral part of the multidisciplinary team who serves as the patient’s nutrition advocate. The RD ensures that the patient’s nutrition needs are being met, including enteral nutrition support initiation and/or management.
  • Primary responsibilities include application of the Nutrition Care Process (NCP) including completion of the nutrition assessment, determination of the nutrition diagnosis, developing and implementing evidence-based nutrition intervention(s), and monitoring and evaluating the patient’s progress, and communicating the nutrition care plan with other members of the care team.
  • The nutrition assessment may include nutrition-focused physical exam, identification and documentation of malnutrition, and care plan development.
  • The RD works independently managing resources in a cost-effective manner, and participates in compliance and quality performance improvement, and serves as a preceptor for dietetic interns.
  • The RD is an integral part of the multidisciplinary team, is involved in education of physicians, residents, medical students, and other health professionals as requested via lectures and in-service opportunities and team conferences and contributes to the development and revision of organizational policies and guidelines.
  • The RD maintains professional competency and skills required for professional practice.

  • Ability to counsel and motivate others.
  • General knowledge of nutrient analysis and Microsoft office.
Education
  • Bachelor’s Degree in Dietetics or related field from a university accredited by the Accreditation Council for Education in Nutrition and Dietetics of the Academy of Nutrition and Dietetics.
  • Master’s degree preferred
  • Completed an ACEND®-accredited supervised practice program/internship at a health-care facility
License, Certification, Registration
  • Registered Dietitian or registry eligible
  • Indiana State License obtained within 6 months of hire
