Job summary
PACE Southeast Michigan is seeking a Registered Dietitian to perform nutrition assessments, develop and implement nutrition care plans, and monitor progress for participants within the IDT. The role emphasizes evidence-based medical nutrition therapy and coordination with caregivers and healthcare professionals.
The position requires a Bachelor’s in dietetics, ACEND accreditation, and RDN credential, with experience in aging populations preferred.
Qualifications
- Bachelor’s degree in dietetics with ACEND accreditation.
- Registered Dietitian Nutritionist (RDN) exam completed.
- Michigan does not require licensure for dietitians at this time.
Responsibilities
- Conducts comprehensive nutrition assessments at enrollment and semi-annually per CMS guidelines.
- Documents PES statements and diagnoses; coordinates plan of care with IDT.
- Provides individualized nutrition education and counseling based on patient needs.
