Job summary

Breaking Ground in Queens is seeking an Assistant Director, Programs, Case Management to supervise and advance case management and housing placement services for street homeless populations.

You will lead clinical coordinators, collaborate with partners, ensure crisis intervention, and promote Housing First and Harm Reduction practices across the program. Strong leadership and coordination across diverse stakeholders are essential.

Qualifications

• NYS LMSW is required and license must be active.
• Minimum of 2 years supervision experience.
• Strong understanding of Housing First and Harm Reduction models.

Responsibilities

• Supervise Clinical Coordinators, Housing Specialists, Housing and Reporting Specialists, and Street Medicine Coordinators
• Provide regular, structured supervision meetings for direct reports
• Ensure high quality service delivery by supporting best practice interventions with street homeless populations and crisis intervention
• Promote effective outreach, case management and placement following Housing First and Harm Reduction models
• Liaise with external stakeholders to ensure responsive program operations
• Provide clinical and operational direction and leadership to the program
• Oversee development and implementation of individual service plans and progress documentation
• Assess and evaluate client functioning
• Supervise and conduct case conferences
• Coordinate delivery of care with multiple service providers
• Support frontline staff in managing difficult situations and de-escalating conflicts
• Perform other duties as assigned
