Responsibilities

• Participate as a member of a multi-disciplinary team providing on-going case management services to adults with severe and persistent mental illnesses.
• Collaborate with housing teams to develop client care and housing retention plans.
• Provide outreach and engagement services to clients to establish and maintain relationships.
• Conduct psychosocial assessments and develop goal planners with consumer participation.
• Coordinate all aspects of consumers' support, including psychiatric and substance abuse treatment.
