DESC seeks an experienced Case Manager within SAGE, DESC's outpatient mental health program. You will provide multidisciplinary case management for adults with severe mental illness, assisting with housing and clinical goals, and delivering crisis intervention and support in collaboration with housing and medical teams.

Qualifications

• Bachelor’s degree in social work, psychology, or related behavioral science.
• Experience with recovery-focused, strengths-based mental health services.
• Knowledge of harm reduction strategies and client advocacy.

Responsibilities

• Participate as a member of a multi-disciplinary team providing ongoing case management for adults with severe mental illnesses.
• Collaborate with housing teams to develop client care and housing retention plans.
• Provide outreach, engagement, crisis intervention, and linkage to community resources.
