DESC is a Seattle-based social services organization focused on homeless services, supportive housing, and behavioral health treatment. It serves thousands with shelter, housing, and comprehensive care coordination.

Case managers work in a multidisciplinary team to develop client-centered care plans, connect clients to community resources, and maintain safety and quality in a dynamic public health setting.

Qualifications

• Bachelor’s degree in social work, psychology, or related behavioral science, OR 1 year of relevant paid work experience.
• Ability to meet Washington DOH requirements for RAAC or superseding credential.
• Ability to drive an agency vehicle with a WA driver's license and insurable record.
• Ability to obtain a WA Food Handler’s Permit.
• Interest in working with individuals difficult to engage and maintain in mental health/substance use programs.
• Be able to pass a Washington State Criminal background check.

Responsibilities

• Participate in psychiatric consultation, supervision, program meetings and trainings.
• Create and implement short-term care plans and discharge dispositions.
• Facilitate client access to community resources and coordinate with external care teams.
• Support a safe, therapeutic environment including 15-minute wellness checks.
• Maintain clinical and administrative records including progress notes and risk assessments.
• Provide transportation and assist with administrative duties as needed.
• Attend onboarding and ongoing trainings.
• Other duties as assigned.
