Job summary

DESC is a leading social services provider focused on homelessness, housing and behavioral health. SAGE intake specialists, case managers, psychiatrists, and nurses support adults with severe mental illness toward recovery, including housing access and stability, clinical care coordination, and community resource linkage.

Join a recovery-focused team operating under a Washington state license and working within King County contracts.

Qualifications

  • Bachelor’s degree in social work, psychology, or related behavioral science, OR a combination of 1 year of relevant paid work experience and demonstration of the ability to perform required job duties.
  • Ability to meet Washington Department of Health requirements for registration as an AAC or equivalent credential.
  • Driver's license and insurable driving record required.
  • Interest in working with clients who are difficult to engage in mental health/substance use programs.
  • Knowledge of Recovery focused, strengths-based work in mental health and Harm Reduction.

Responsibilities

  • Participate on a multi-disciplinary team providing ongoing case management to adults with severe mental illness.
  • Develop client care and housing retention plans with housing teams.
  • Provide outreach, engagement, assessments, and goal planning with consumer participation.
  • Coordinate psychiatric and substance abuse treatment, 24-hour support, and acute case management.
  • Advocate for consumers’ access to community resources; maintain professional relations with landlords and housing staff.
  • Travel locally for outreach, home visits, inspections, and address housing barriers.
  • Provide crisis intervention and risk assessment; deliver counseling as qualified.
  • Maintain clinical and administrative records and participate in quality assurance reviews.
