Job summary

DESC, in partnership with SAGE, seeks a dedicated Case Manager to support adults with complex mental health needs through comprehensive case management, housing coordination, and community resource linkage.

You will engage clients, conduct assessments, and coordinate medical and behavioral health services, while advocating for stable housing and wellbeing. The role requires collaboration across teams and may involve local travel in the Seattle area.

Qualifications

  • Bachelor’s degree in social work, psychology, or related behavioral science, or 1 year of relevant experience.
  • Ability to drive and possess a WA driver’s license.
  • Bilingual in Spanish and English (preferred).
  • Interest in working with clients with complex needs.

Responsibilities

  • Participate as a member of a multi-disciplinary team providing ongoing case management for adults with complex behavioral health needs.
  • Collaborate on client care and housing retention plans with Scattered Sites Supportive Housing.
  • Provide outreach to engage clients and maintain relationships.
  • Conduct psychosocial assessments and develop goal planners with consumer input.
  • Coordinate psychiatric and substance use services, 24-hour supports, and access to housing and essentials.
  • Advocate for clients to access community resources and maintain rights.
  • Travel locally for outreach, home visits, and housing inspections.
  • Provide crisis intervention and risk assessment; deliver counseling as qualified.
  • Serve as protective payee when needed, budgeting and distributing funds.
  • Build relationships with landlords and property managers to support leases.
