Job summary

DESC, a leader in homeless services, seeks a Case Manager under SAGE to provide comprehensive case management to adults with severe mental illness. You will coordinate psychiatric and substance abuse treatment, support housing retention, and advocate for client resources, while engaging in community partnerships.

The role requires a social work–related degree, state credentialing eligibility, and a valid driver’s license.

Qualifications

  • Meet Washington credential requirements (AAC or equivalent).
  • Bachelor’s degree in social work, psychology, or related field.
  • Valid driver’s license and insurable driving record.
  • Interest in working with challenging clients.
  • Knowledge of recovery-focused and harm-reduction approaches.
  • Ability to work with diverse staff.

Responsibilities

  • Participate as a member of a multi-disciplinary and multi-departmental team providing on-going case management services to adults with severe and persistent mental illnesses.
  • Collaborate with DESC’s Scattered Sites Supportive Housing team in the development of client care and housing retention plans.
  • Provide outreach and engagement services to assigned clients, to initially engage clients and maintain working relationships.
  • Conduct psychosocial assessments; develop goal planners with consumer participation; facilitate linkages to collaborative resources when appropriate.
  • Assume primary responsibility for coordinating all aspects of consumers' support and goal planners; provide and/or coordinate psychiatric and substance abuse treatment; 24-hour support services; and acute case management.
  • Advocate for consumers' access to community resources and services; consult with community providers to ensure continuity of care.
  • Travel locally to outreach consumers; make home visits and assist with inspections and renewals; address housing retention barriers.
  • Provide crisis intervention and risk assessment; tolerate unusual or unpleasant behavior.
  • Provide counseling, as qualified.
  • Fulfill role of protective payee; create monthly budgets with consumers and assist with purchases.
  • Establish and maintain professional relationships with landlords and property managers for lease compliance.
  • Document services and adhere to administrative deadlines.
  • Support clients in achieving and maintaining healthy living conditions.
  • Participation in psychiatric consultations, supervision, program meetings and in-service trainings; clinical reviews and case conferences for caseload.
  • Attend trainings related to landlord/tenant laws, housing vouchers, and related topics.
  • Comply with clinical accountability policies; maintain clinical and administrative records; participate in quality assurance reviews.
  • Comply with program research and evaluation procedures.
  • Other duties as assigned.
