Mainline Health Systems, Inc. is seeking a Clinical Case Manager to join the care team under the Clinic Manager. This role supports a Patient Centered Medical Home model and collaborates with all health services to deliver comprehensive patient care. You will participate in continuous quality improvement and maintain professional communication daily. Typical hours are 8 a.m.

to 5:30 p.m. Monday–Thursday and 8 a.m. to 12:00 p.m. on Friday; schedule adjustments may be required.

Competenze

• One year of experience in a medical field/setting.
• Unencumbered Nursing License (RN) is required.
• Ability to communicate clearly and work well in a team.

Mansioni

• Handle clinic telephone encounters and patient inquiries.
• Process refill requests and lab result notifications.
• Manage electronic messages and coordinate with staff.
• Attend monthly clinical staff meetings and participate in daily huddles.
• Support workflow aligning with patient-centered care model.
