Job summary

RHA Health Services, LLC is hiring a Clinical Case Manager in New Jersey to coordinate IDT meetings, monitor ISP implementation, and streamline admissions. You will analyze progress, maintain communication with the Support Coordinator, and conduct site visits to ensure policy and data integrity.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required; Master’s degree a plus
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with Master’s degree
• Working knowledge of applicable regulations that apply to our service offerings
• Valid driver’s license, auto insurance, and reliable transportation
• Supervisory/management experience and experience writing person-centered (individualized service) plans preferred
• Must be at least 18 years of age.

Responsibilities

• Assist with coordinating IDT meetings and the ISPs
• Supervises and monitors the implementation of the ISP
• Ensure a streamlined admission process
• Responds to incident reports through collaboration with staff to develop root cause analyses and response plans
• Analyzes and summarizes progress status by determining program appropriateness and recommending changes
• Communicates regularly with the Support Coordinator and other circle of support members if amendments to the ISP are needed
• Documents all communication
• Regularly spends time at the location where services are performed
• Conducts regularly announced and unannounced visits in residential homes to ensure policy and program data are in place
• Provides coverage if shortages of direct support personnel but continues to be responsible for management duties
• Trains and conducts in-services for direct care staff
• Conducts internal peer reviews, observations, and other QA assessments
• Conducts CQL’s Personal Outcome Measures
• Supports health record components for accuracy and timeliness
• All other duties as assigned
