Qualifications

• Must be 18+ years old with a High School Diploma or equivalent.
• Valid New Jersey driver’s license and driving record acceptable to insurance and regulatory requirements.
• Ability to follow written and verbal instructions and communicate clearly with clients and staff, using agency databases.
• Willingness/ability to work a flexible schedule to meet residents’ needs.

Job Details

The Case Management Specialist will support the case management department specifically in the provision of services and supports to individuals with intellectual or developmental disabilities for planning, organizing, coordinating, and facilitating health care related activities. This includes monitoring electronic health records, medical appointments, critical logs, and other relevant documentation to ensure information is up to date and accurate. This role provides guidance and support to the agency and planning teams to ensure quality standards are consistently met and healthcare needs are satisfied routinely and as needed.

Qualifications

• Must be at least 18 years old and have a High School Diploma or equivalent.
• Current driver's license valid in the state of New Jersey and driving record acceptable to insurance and regulatory requirements.
• Must possess basic skills for following written and verbal instruction, and for communicating client- and job-related issues orally and in writing, including the agency electronic data base systems. Must have the ability to communicate with individuals with whom they are working.
• Must be willing and able to work a flexible schedule as assigned or established to ensure needs of the individuals are met.
