Alternatives, Inc. in Raritan, NJ is seeking a Mental Health Case Manager to deliver in-home and on-site services aligned with individualized treatment plans.

You will support clients in areas like motivation, money management, daily living skills, and housing assistance, coordinating with the treatment team.

Qualifications

• Bachelor's degree in human services related field is required.
• Experience working with individuals who have mental illness.
• Experience with electronic health record systems and Medicaid reimbursement a plus.
• Ability to pass criminal/registry background checks.
• Successful completion and passing drug screen.
• Clear tuberculosis screening.
• Valid driver's license with no more than 5 points and no DUI in last 3 years.

Responsibilities

• Provide in-home and on-site services per each individual's treatment plan.
• Evaluate skills and needs and make recommendations to the treatment team.
• Learn the 8 dimensions of wellness and interventions to provide Medicaid services.
• Use an EHR to write clear progress notes for Medicaid billing.
• Provide crisis intervention and crisis prevention plans.
• Pass all agency trainings.

Job Location: LEGAL ADDRESS - RARITAN, NJ, Position Type: Full Time, Salary Range: $28.00 - $28.00Hourly, Job Shift: Day Shift

DESCRIPTION

Community Support Services is a clinical case management program serving individuals diagnosed with mental illness, who live independently in the community. Clients live in either Agency owned housing or their own residences. Case Managers work with the individuals on their personal goals in order to meet their needs and enhance their quality of life.

Essential Duties and Responsibilities

• Provide in home and on-site services and support as per each individual's treatment plan; including provision of direct service to clients in areas such as motivational interviewing, money management skills, daily living skills, symptom improvement, psycho-education, housing support, job skills training, etc.
• Evaluate skills and needs of each individual and make recommendations to treatment team
• Learn the 8 dimensions of wellness and multiple interventions in order to provide quality Medicaid services
• Utilize an electronic health record (EHR) system to write clear and concise progress notes to bill for Medicaid reimbursable services
• Provide crisis intervention services, as necessary, and crises prevention plans in an effort to improve crises response
• Pass all Agency and program specific trainings

Training

• All employees are required to complete 10-15 trainings upon hire and on an ongoing basis
• Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting
• All trainings are paid

Mental Health Case Manager

Must possess valid NJ/NY/PA driver's license

In order to be eligible for employment, applicant must not have more than 5 points on their driving record and no DUIs within the last 3 years.
