Job summary

Beacon Hill in Boston, Massachusetts seeks a Clinical Case Manager to provide case management and supportive counseling to individuals experiencing homelessness with co-occurring mental health and/or substance use concerns. You will build supportive relationships, connect clients with resources, and promote housing stability and personal goals.

Key duties include biopsychosocial assessments, individualized service plans, progress notes, crisis intervention, and coordinated referrals.

Qualifications

  • Bachelor's degree in a human services-related field; relevant experience may be considered in lieu of a degree.
  • 3+ years of experience in case management and working with individuals experiencing substance use disorders, mental health concerns, or other complex needs.
  • Knowledge of Harm Reduction, Motivational Interviewing, Relapse Prevention, Trauma-Informed Care, and other evidence-based service models.
  • Experience with crisis intervention and de-escalation, including the ability to exercise sound clinical judgment and maintain professional boundaries.
  • Strong written and verbal communication skills, including the ability to complete clear progress notes and service plans.
  • Strong computer and electronic recordkeeping skills.
  • LADC or CADC credential is a plus.

Responsibilities

  • Work collaboratively with participants toward permanent housing, increased income and benefits, greater stability, and improved self-determination.
  • Respond to crisis situations and provide appropriate intervention and de-escalation while helping maintain a safe environment.
  • Coordinate community-based referrals and provide appointment accompaniment when needed to support continuity of care and independent living.
  • Collaborate with clinical and operational teams while participating in case conferences, program meetings, and other required meetings.

Position Overview
