Job summary

Qualifications

• Graduate from an accredited school of nursing.
• BSN required; MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Current RN licensure in the state.
• Case management certification recommended within 2 years of employment.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on discharge needs.
• Assesses all relevant data and obtains information by interviewing patient/family and evaluating patient needs.
• Ensures that a Plan of Care is developed and implemented with nursing to improve outcomes and cost efficiency.
• Collaborates with patient, caregivers, the multidisciplinary team and community service providers to execute the Plan of Care.
• Coordinates the discharge planning process; ensures discharge planning needs are completed.
• Monitors and adjusts the Plan of Care and communicates with providers and payors.
• Documents Case Management actions in the EMR and communicates necessary information to providers.
