Qualifications

Responsibilities

• Identify patients who would benefit from Case Management interventions after screening.
• Assess data by interviewing patients and families to evaluate needs.
• Complete psychosocial assessments per standards to identify emotional and social needs.
• Develop and implement psychosocial treatment plans with appropriate modalities.
• Initiate internal and external referrals for patient needs.
• Maintain knowledge of federal, state, and local regulations impacting care.
• Advocate for unmet patient needs on behalf of patients and families.
• Contribute to discharge planning with multidisciplinary teams.
• Stay informed about payer and reimbursement practices affecting care plans.
• Coordinate patient and family care conferences as needed.
• Identify barriers to care and collaborate on elimination strategies.
• Document in EMR per departmental standards.
• Guide patients through post-discharge options and payer considerations.
