• Completes psychosocial assessments to identify emotional, social and environmental needs.
• Develops and implements psychosocial treatment plans including crisis intervention, individual and family therapies, and bereavement counseling.
• Initiates internal and external referrals for individual patient needs.
• Documents in the Electronic Medical Record per departmental standards.
