Job summary

CVS Health is seeking a licensed Behavioral Health Clinical Case Manager to support members remotely, Monday–Friday, with no holidays. You will conduct assessments, develop care plans, and coordinate with providers to improve outcomes.

This full-time role requires a Master's in Behavioral Health and an active license in your state, plus 3+ years of post-master’s experience. Strong documentation, telehealth proficiency, and cost-aware care are essential.

Qualifications

• Master's degree in Behavioral Health or related field.
• Active, unrestricted license in your state (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
• 3+ years of post-master's direct clinical experience in relevant settings.
• Experience treating behavioral health and substance use disorders.
• Strong communication and documentation skills in clinical systems.

Responsibilities

• Conduct comprehensive behavioral health assessments using tools, data analysis, and benefit plan reviews.
• Develop individualized care plans addressing behavioral and physical health needs.
• Coordinate care with providers, organizations, and community resources.
• Apply clinical judgment to identify and mitigate risk factors affecting outcomes.
• Provide crisis intervention and ensure appropriate referrals and follow-up.
• Coordinate psychosocial wraparound services and access community resources.
• Support cost-effective care through proactive planning and utilization.

Clinical Case Manager – Behavioral Health (Licensed)

Are you a licensed behavioral health clinician who thrives on helping members navigate complex mental health and substance use challenges? Join our team and use your clinical expertise to provide comprehensive case management, crisis intervention, and care coordination that improves outcomes and supports whole-person health.

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews
• Develop individualized care plans that address both behavioral and physical health needs
• Coordinate care with providers, community organizations, and support services
• Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes
• Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up
• Coordinate psychosocial wraparound services and connect members with community resources
• Support cost-effective care through proactive care planning and resource utilization
• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field
• ~ Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent)
• ~3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting
• ~3+ years of experience treating mental health and substance use disorders
• ~Case Management experience
• ~ Ability to simultaneously talk on the phone and document in clinical systems
• ~ Strong technical proficiency and comfort working in a virtual environment

• Managed Care experience
• Utilization Review or Utilization Management experience
• Discharge Planning experience
• Experience working with complex behavioral health populations
• Fully remote position available anywhere in the United States
• Consistent Monday–Friday schedule
• No weekends or holidays
• Meaningful work supporting members with complex behavioral health needs

The typical pay range for this role is:

$This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
