Veteran Transition Services (VTS) provides general case management services for Veterans who have previously experienced homelessness and are now residing in permanent housing. Veteran Transition Services offers support in many domains in order to help Veterans achieve financial independence and long‑term stability. It is important to note that Veterans who are already enrolled in other programs such as HUD/VASH or SSVF are generally ineligible for this program. Services include case management and assistance with budgeting, maintaining housing, accessing resources, broadening social networks, building life skills, and increasing income through benefits and/or employment. Individualized service plans are developed. Veterans have 24/7 access to a qualified mental health professional through our Veterans hotline. Support and referrals are also provided to Veterans.
Full‑time, in‑office position and requires a 40‑hour week on average. This is a Monday through Friday, 9 to 5 position. This position reports to the Director of Resident and Clinical Services. This position includes an on‑call component.
$58,000–$62,000 (commensurate with education and experience)
Physical demands include sedentary work, operating a computer and other office productivity machinery with frequent communication with clients and colleagues of VOA Illinois and others involved in the care and custody of clients. The individual hired must be able to move, traverse and travel in the community in a vehicle, assist clients in and out of a vehicle if necessary and transport clients in the community. This position operates in a professional office environment and is also required to work from multiple locations throughout the greater Chicagoland area to complete home visits, outreach, and attend events. This position will require 40 hours a week on average and will be variable and intermittent. Includes weekend and evening hours. Also includes an on‑call responsibility for 7 days biweekly.
