VOA Illinois is seeking a Client Support & Service Coordinator for the Housing is Recovery HiR program in Elgin, IL. The role provides comprehensive case management to help participants access stable housing and recovery supports.

This full-time, in-office position requires travel within the community, a valid driver's license, and the ability to lift up to 50 lbs. You will document interactions in EchoVantage and HMIS, collaborating with partners to ensure timely, cost-effective services.

Client Support and Service Coordinator-Housing is Recovery (HiR)

Location: Elgin, IL 60120

Pay: $22.20 - $26.64 per hour ($46,176 - $55,411 annually)

Schedule: Full Time, Non-Exempt (40 hours/week)

Work Arrangement: On-site (hybrid eligible after 6 months)

Program Overview:

The Housing is Recovery (HiR) program provides housing and recovery support for individuals with serious mental illness or substance use disorders who face high risks related to homelessness, institutionalization, overdose, or repeated hospital or justice system involvement. The program combines bridge rental subsidies with coordinated recovery services and treatment, ensuring participants can access stable housing while engaging in long-term recovery supports.

Position Summary:

The Client Support & Service Coordinator (CSSC) provides comprehensive case management for clients whose needs are not fully met by the general service community, including assessing social, psychological, and physical needs and developing individualized service plans. Offers ongoing support and progress monitoring, delivering services with cultural and socioeconomic sensitivity. Serves as a liaison between clients, community agencies, and service providers to ensure coordinated, timely, and appropriate services.

Work Environment & Schedule:
  • Full-time, in-office position averaging 40 hours per week
  • Community-based travel required
  • Valid driver’s license required
  • Ability to lift up to 50 lbs.
  • Physical demands include sedentary work, computer use, and frequent communication with clients, colleagues, and partners involved in client support.
  • Must be able to travel within the community, transport clients when necessary, and assist clients in and out of vehicles.
  • This position reports to the Program Manager for the HiR program.
What You’ll Do:
  • Provide case management (evaluation of social/psychological/physical needs, and development of a service plan) when the general service community is not providing such service.
  • Meet with program participants to provide support, monitor progress on the service plan, etc.
  • Deliver services with sensitivity to the clients’ cultural and socioeconomic characteristics.
  • Act as a liaison between community agencies, service providers, and clients.
  • Use the least intrusive intervention necessary to alleviate a problem situation.
  • Facilitate and monitor the delivery of appropriate, timely, and satisfactory services.
  • Educate clients on service availability, application procedures, client rights, etc.
  • Advocate and may negotiate on clients’ behalf for adequate, timely & cost effective service provision.
  • Meet with service providers as needed and appropriate.
  • Pursue avenues for additional services through private local, state, and federal sources.
  • Document contact with clients, providers, and families.
  • Use databases such as EchoVantage and HMIS for data entry.
  • Uphold the mission and values of VOA Illinois at all times.
Qualifications
Required:
  • Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or related field.
  • Demonstrated experience in the helping/service field, including service management such as organizing, problem solving and advocating.
  • Experience and demonstrated proficiency/success in leveraging and working with community resources.
  • Strong communication, collaboration, and conflict de-escalation skills.
  • Proficiency in Microsoft Office and electronic documentation systems.
Preferred:
  • Knowledge of local social service and behavioral health resources
  • Experience with advocacy, intake, care coordination, or crisis intervention
