Job summary

Providence in Alaska is seeking a Clinical Care Coordinator to join the multidisciplinary care team. You will coordinate services for panel patients, help those who struggle navigating the system, manage specialty referrals, and support self-care and community resource access.

You will collaborate with outside providers to ensure seamless transitions of care, educate patients, and connect them with applicable resources.

Qualifications

• Associate's Degree or equivalent educ/experience.
• 2 years Healthcare experience.
• Experience in an outpatient setting.
• Experience in Case Management.

Responsibilities

• Coordinate services for panel patients and assist those who struggle to navigate the healthcare system.
• Manage specialty referrals and support patients in self-care and accessing community resources.
