Job summary

Qualifications

• Associate's Degree or equivalent educ/experience
• 2 years Healthcare experience.
• Experience in an outpatient setting.
• Experience in Case Management.

Responsibilities

• Manage specialty referrals and follow-up.
• Support patients in self-care and identify community resources.
• Outreach to outside providers to ensure seamless transitions of care.
