Job summary

Providence Health & Services in Anchorage is seeking a Clinical Care Coordinator to join PMGA's multidisciplinary team. You will coordinate services for panel patients, focus on navigation challenges, manage referrals, and support patients in self-care while connecting them with community resources.

You will collaborate with outside providers to ensure seamless transitions of care, provide patient education, and assist with access to applicable resources.

Qualifications

• Associate's Degree or equivalent educ/experience
• 2 years Healthcare experience.
• Experience in an outpatient setting.
• Experience in Case Management.

The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key roles are coordination of services for panel patients, focusing on those who face particular difficulty with navigating the healthcare system, management of specialty referrals and supports patients in self-care and identifying relevant community resources. The Clinical Care Coordinator collaborates with and outreaches to the outside service providers to provide seamless transitions of care, and provides patient education while assisting with access to applicable resources.
