Your Health in Macon is seeking a Clinical Coordinator to support all clinical needs for our local patients. The role emphasizes chart preparation, scheduling visits, managing chronic care activities, and providing provider and patient support.

This is a full-time, 8:00 AM to 5:00 PM position based in the Macon office, with responsibilities across facilities and home/clinic visits as needed. The successful candidate will have CMA or nursing licensure, CPR, and strong communication and teamwork

Qualifications

• Certified Medical Assistant (CMA) or licensed nurse in good standing.
• Minimum of 1 year experience in a healthcare setting preferred.
• Current CPR certification.
• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Ability to foster a cooperative work environment.
• Team player with ability to manage multiple responsibilities and sound judgment.
• Ability to read and communicate effectively.
• Basic computer knowledge.
• Willingness to travel between offices.

Responsibilities

• Review and organize patient medical records for visits.
• Ensure documentation, test results, diagnoses, and relevant information are accessible for the provider; use Navina.
• Maintain confidentiality and verify consent forms and alignment paperwork.
• Reconcile medications.
• Coordinate with the care team to schedule patient appointments with providers.
• Communicate with patients via email, portal, phone, text, or in person to schedule visits and provide pre-appointment instructions.
• Use communication tools such as phone, email, and Microsoft Teams.
• Document and log care management activities.
