Your Health in Savannah, GA is seeking a Clinical Coordinator to manage core clinical tasks, chart prep, scheduling, and care coordination. You will support providers and patients to ensure timely, accurate care delivery across facilities and virtual visits.

The role requires CMA or licensed nurse credentials, CPR certification, and at least 1 year in a healthcare setting. This is a Monday–Friday, 8:00 AM–5:00 PM full-time position based in Savannah.

Qualifications

• Must be CMA or licensed nurse (LPN/RN) in the state of employment.
• CPR certification required; healthcare setting experience preferred.
• High school diploma or equivalent required.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all documentation and test results are accessible for the provider; update Navina as needed.
• Maintain confidentiality and ensure consent forms and alignment paperwork are completed.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team.
• Interact with patients via multiple channels to gather information and provide pre-appointment instructions.
• Log and document care management activities accurately.
