Job summary

Christian Community Health Center is seeking a Licensed Practical Nurse to join the Care Coordination team. The LPN Care Coordinator will monitor and coordinate care for patients needing chronic disease management, under supervision of the Director of Nursing and CMO.

The role requires EHR proficiency, CQI experience, and the ability to work across multiple locations with varied schedules to meet patient needs.

Qualifications

• Graduate of an accredited nursing program.
• Current BLS certification.
• At least 1 year of experience with electronic health records (EHR).
• Ambulatory/clinic experience in care coordination or similar nursing duties.
• Detail-oriented, organized, and able to work in a fast-paced environment with excellent customer service.
• Willing to work across multiple CCHC locations with varied schedules.
• Experience with CQI tracking and documentation.
• Familiarity with Vaccines for Children (VFC) program.

Responsibilities

• Develop and implement standards, protocols and processes to support care coordination and chronic care management.
• Create, implement and maintain documented workflows and drive process improvement.
• Supervise Care Coordination Team to achieve optimal patient outcomes.
• Identify patients for care management via referrals or data criteria.
• Perform patient outreach and scheduling for care planning as needed.
• Track and schedule follow-ups for recently discharged patients.
• Engage patients and providers in goal setting and care planning.
• Ensure documentation is completed in the EHR.
• Collaborate with care team to resolve care gaps and barriers.
• Maintain HIPAA-compliant patient records and communications.
