Qualifications

• Licensed master's level clinician.

Responsibilities

• Completes intake assessments to identify patient needs and goals and develops a treatment plan.
• Identifies community resources and connects patients and families with outpatient services and higher levels of care.
• Monitors progress of care coordination through scheduling, outreach, and collaboration with community providers.
• Provides supportive counseling and aids in achieving patient goals.
• Schedules, facilitates, and documents therapy sessions for patients.
• Offers post-crisis support and updates safety plans as needed.
• Acts as liaison between health system resources and community services for patients and families.
• Demonstrates professionalism, planning, teamwork, and adaptability in daily tasks.
