Job summary

Advocate Health is seeking a licensed mental health professional to provide comprehensive counseling and case management services in a clinical setting. The role involves collaborating with guardians and community agencies, assessing patient needs, and writing individualized treatment plans across age groups.

Candidates should have a Master’s degree in a relevant field, state licensure or certification within 6 months, CPI and CPR training within 90 days, and strong therapeutic communication

Qualifications

• Master's degree in a relevant human services field from an accredited school.
• Associate licensure and/or certification to be issued within 6 months of employment.
• CPI and CPR training must be completed within 90 days of hire.
• Clinical experience preferred; case management experience with Serious and Persistent Mental Illness and/or substance abuse preferred.
• Must have good therapeutic communication skills.

Responsibilities

• Collaborate with guardians and community agencies to meet patient care needs.
• Assess patients' problems, needs, and functioning level.
• Write treatment plans based on diagnosis, age, and clinical presentation.
• Provide psychotherapy using various modalities and development principles.
• Document patient care accurately in the medical record.
• Present cases in clinical team meetings clearly and concisely.
• Coordinate discharge planning and plan of care with the next provider.
