Job summary

Skywood Recovery Center, part of Universal Health Services, located in Augusta, MI, seeks an Assessment Coordinator to provide clinical oversight of the Assessment area and coordinate the assessment process for all prospective patients, including detox patients.

The role involves conducting biopsychosocial assessments, determining levels of care, coordinating admissions, and ensuring timely documentation while maintaining regulatory compliance.

Qualifications

• Bachelor's degree required.
• 1–3 years of related experience required; 3–5 years preferred.

Responsibilities

• Oversees daily workflow of the Assessment Department and coordinates assessments, admissions, detox intakes, and patient flow.
• Conducts comprehensive biopsychosocial assessments and evaluates patients for appropriate level of care.
• Coordinates admission documentation, consents, releases, and clinical records for regulatory compliance.
• Provides crisis intervention, risk assessment, suicide assessments, and de-escalation support for assessment and detox patients.
• Coordinates communication between providers, nursing, UM, and clinical staff to facilitate timely admissions.
• Provides patient orientation and explains program expectations, rights, and treatment processes.
• Reviews documentation for quality, accuracy, and timeliness with compliance to Joint Commission, state, and payer requirements.
• Participates in quality improvement, staff education, policy development, and process improvement.
• Performs other duties as assigned.
