Job summary

Skywood Recovery Center seeks an Assessment Coordinator to oversee the assessment department and coordinate admissions for detox and other patients.

You will conduct biopsychosocial assessments, determine levels of care, and ensure timely documentation while coordinating with physicians, nursing, and clinical staff to promote safe, compliant admissions.

Qualifications

  • Bachelor’s degree in a health or human services field.
  • 1–3 years of experience in assessment or admissions.
  • Knowledge of ASAM Criteria and regulatory standards.
  • Experience in detox or behavioral health settings preferred.
  • Strong communication and crisis management abilities.

Responsibilities

  • Oversees the daily workflow of the Assessment Department.
  • Conducts comprehensive biopsychosocial assessments and determines level of care.
  • Coordinates admission documentation and releases for regulatory compliance.
  • Provides crisis intervention and risk assessments for patients.
  • Collaborates with physicians, nursing, UM, and clinical staff.
  • Ensures regulatory and payer requirements are met during admissions.
  • Participates in quality improvement and staff education initiatives.
  • Performs other duties as assigned.

Qualifications
Position Summary

The Assessment Coordinator provides clinical oversight of the Assessment area and coordinates the assessment process for all prospective patients, including detox patients. This position is responsible for conducting comprehensive biopsychosocial assessments, determining appropriate levels of care, coordinating admissions, managing crisis situations during the intake process, and ensuring all required clinical and regulatory documentation is completed accurately and timely. The Assessment Coordinator serves as a liaison between physicians, nursing, UM, and clinical staff to facilitate safe, efficient admissions while promoting exceptional patient care and regulatory compliance.

Specialized Skills And Knowledge
  • Thorough knowledge of substance use disorders, detoxification, co-occurring disorders, and behavioral health treatment.
  • Strong clinical assessment, crisis intervention, suicide risk assessment, and de-escalation skills.
  • Knowledge of ASAM Criteria and level of care determinations.
  • Knowledge of Joint Commission, CMS, HIPPA, state licensing regulations, and behavioral health documentation standards.
  • Excellent verbal, written, and interpersonal communication skills.
  • Strong understanding of motivational interviewing and trauma-informed care.
  • Ability to prioritize multiple assessments and admissions while responding effectively during crisis situations.
  • Ability to effectively manage behavioral excalations and support staff during crisis situations.
  • Knowledge of patient supervision, safety monitoring, and incident reporting requirements.
  • Strong organizational skills with the ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
Essential Job Duties/Responsibilities
  • Oversees the daily workflow of the Assessment Department, coordinating assessments, admissions, detox intakes, and patient flow to ensure efficient operations.
  • Conducts comprehensive biopsychosocial assessments, evaluates patients for appropriate level of care, and collaborates with providers regarding admission recommendations.
  • Coordinates admission documentation, consents, releases, and clinical records to ensure compliance with regulatory and organizational standards.
  • Provides crisis intervention, risk assessment, suicide assessments, behavioral de-escalation, and clinical support for assessment and detox patients while ensuring patient and staff safety.
  • Coordinates communication between providers, nursing, UM, and clinical staff to facilitate timely admissions and continuity of care.
  • Provides patient orientation, explains program expectation, patient rights, treatment processes, and facilitates a smooth transition into treatment services.
  • Reviews documentation for quality, accuracy, and timeliness while ensuring compliance with Joint Commission, state, and payer requirements.
  • Participates in quality improvement initiatives, staff education, policy development, and departmental process improvement activities.
  • Performs other duties as assigned/required by this position.
Primary Equipment/Applications Used

Computer/Proficient in Microsoft Office Suite. Experience utilizing EMR and timekeeping software preferred.

Bloodborne Pathogens Exposure

This position may involve potential exposure to blood or bodily fluids. The employee is expected to follow all established infection control and personal protective equipment protocols.
