Job summary

Rise Recovery seeks an Assessment Specialist to serve as a licensed clinician handling clinical assessments, intake, and admissions. You will evaluate substance use, mental health, suicide risk, and other needs to determine appropriate care and connect individuals to the right program or external level of care.

You will lead intake processes, document findings in the EHR, and collaborate with care teams to develop initial treatment recommendations.

Qualifications

• Must hold LCDC license in good standing in Texas.
• Minimum of three years in substance use disorder treatment, behavioral health, or related field.
• Experience conducting clinical assessments, screenings, or referrals based on presenting needs.
• Knowledge of ASAM criteria, DSM-5 classifications, and TX requirements.
• Strong clinical interviewing, documentation, and communication skills.

Responsibilities

• Conduct comprehensive clinical assessments and intake interviews to evaluate needs and eligibility.
• Assess for suicide risk, self-harm, overdose risk, withdrawal, and other safety concerns; initiate crisis procedures if needed.
• Apply ASAM criteria and DSM-5 classifications to determine level of care and needs.
• Determine disposition and coordinate referrals internally or externally as appropriate.
• Facilitate admission and intake with complete clinical documentation in the EHR.
• Develop initial treatment recommendations and communicate them to the care team.
• Educate participants and families about outcomes, services, and next steps.
• Collaborate with clinicians and referral sources to ensure appropriate placement and transitions.
• Maintain timely, accurate documentation in the EHR for assessments, risk screenings, and referrals.
