Job summary

You will conduct on-site visits, review medical records, coordinate with admissions, generate tours, manage referrals, and contribute to social media and community events.

Responsibilities

• Generate new referrals and maintain hospital relationships with physicians and social workers.
• Develop and execute a marketing strategy to increase occupancy across three communities.
• Assess patient eligibility for Skilled Nursing/Long-Term Care via in-person assessments and records review.
• Coordinate with admissions staff to ensure smooth transitions and complete documentation.
• Schedule tours and engage with patients, families, and referral sources.
• Contribute to social media and community events, and work with Marketing teams.
