Job summary

Engage Healthcare in Silver Spring is seeking a Clinical Liaison to lead and coordinate the referral process for the local market. You will conduct timely clinical and financial evaluations of patient referrals, assess needs, and direct referrals to appropriate facilities.

The role includes developing solutions to meet identified needs and drive referrals to selected facilities. Essential duties include generating referrals, building relationships with facilities and sources, and ensuring timely

Qualifications

• Three or more years of experience in long-term care or related industry.
• One to two years of sales and/or marketing experience a plus; case management preferred.
• Preference given to candidates with a nursing background.
• Understand long-term care business, census and quality mix goals, and referral expectations.
• Effective verbal and written English communication skills.
• Proficient in MS Word, Excel, PowerPoint and Outlook; strong computer skills.

Responsibilities

• Generate referrals to selected facilities to support census and quality mix goals.
• Assist in building relationships with facility and referral sources.
• Respond timely to inquiry calls from hospital discharge planners and other referral sources.
• Conduct patient assessments promptly after referral, aiming for completion within 30 minutes.
• Assist with pre-admission care planning as needed.
• Coordinate with facility staff to understand census, quality mix, and bed management needs.
• Provide daily updates to Admissions & Marketing Directors on referrals and admissions.
• Maintain relationships with Directors of Nursing and Business Office Managers for admission decisions.
• Make admissions recommendations to facility management and communicate room/bed needs.
