Job summary

AccentCare in Newark, Delaware is seeking a full-time Clinical Liaison RN to support hospice admissions and patient transitions.

The role blends nursing care with business outreach, coordinating with physicians, admissions, and hospice teams to ensure timely, compassionate support.

Requirements include active RN license in Delaware, 1 year local RN experience, a valid driver’s license with auto insurance, and a commitment to caring for patients and families.

Qualifications

• 1 year RN experience in local market with knowledge of home health regulations and Medicare requirements.
• Previous medical sales experience preferred.
• Licensed RN currently licensed in the state of operation.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Develop and implement individualized marketing plan with Hospice Care Consultants.
• Increase relationships with contracted inpatient facilities and physician groups.
• Liaise with Hospice Medical Director and physicians on behalf of providers and patients/families.
• Make inpatient visits to contracted facilities and coordinate with Admissions and Team Directors on care transitions.
