Job summary

Acacia Network is seeking an Admissions and Outreach Manager in Brooklyn, NY. The role provides full clinical oversight for admissions and peer staff within the outpatient program, working with a multidisciplinary team to ensure timely intakes and comprehensive service delivery.

The position requires LMSW or MSW eligibility, licensure or equivalent QHP, and at least 3 years in addiction/counseling with supervisory experience.

Qualifications

QHP required: LMSW, MSW eligible for licensing.
Unrestricted OASAS CASAC or another equivalent QHP license.
Minimum of 3 years in the addiction or counseling field, supervisory experience preferred.
Working knowledge of NextGen Electronic Health Record (EHR).
Excellent organizational, communication and time management skills.

Responsibilities

Review prospective clients for outpatient admission; coordinate intakes and medical appointments.
Participate in case reviews and conferences to support client compliance and progress.
Conduct substance use and mental health assessments using standardized tools.
Collaborate with the treatment team to ensure a comprehensive service plan.
Supervise Peer Specialists and complete staff evaluations and performance plans.
Coordinate and present to criminal justice agencies, hospitals and other programs to strengthen referrals.
