Job summary

Sedgebrook is seeking a Clinical Admissions Liaison to drive occupancy growth by managing referrals, evaluating admission appropriateness, and coordinating with hospitals and community partners. This role ensures timely admissions and smooth resident transitions with a strong focus on customer service.

You will review referrals, conduct clinical assessments, and collaborate with clinical and operations teams while participating in on-call and weekend coverage.

Qualifications

• Active clinical license required: RN, LPN, PT, PTA, OT, COTA, ST, or Clinical Social Worker.
• Bachelor's degree preferred.
• 3-5 years of clinical experience in acute care, post-acute care, or senior living.
• Strong clinical assessment, critical thinking, and decision-making skills.
• Proven ability to build referral relationships and achieve occupancy goals.
• Experience with EMR/CRM systems; PointClickCare experience preferred.
• Excellent communication, organization, and customer service skills.
• Proficiency with Microsoft Office.
• Valid driver's license and reliable transportation.
• Ability to work independently, manage multiple priorities, and maintain a flexible schedule.

Responsibilities

• Achieve occupancy and census goals.
• Review referrals and clinical documentation to determine admission eligibility.
• Conduct clinical assessments and coordinate admissions with internal teams.
• Ensure smooth transitions between hospitals, skilled nursing, and other levels of care.
• Triage hospitalized residents and coordinate appropriate placement or return to care.
• Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, and other referral sources.
• Collaborate with admissions, clinical, and operations teams on referral and census management.
• Support outreach events, marketing initiatives, and community engagement efforts.
• Assist with training and guidance of Admissions Coordinators.
• Serve as backup to the Admissions Director and provide coverage as needed.
• Participate in on-call, weekend, and emergency response responsibilities.
