• Manage 3 skilled nursing facilities admissions in Palmyra and Lititz, PA.
• Manages admission process from referral to facility transfer.
• Represents and promotes public relations and Interacts with referral sources including hospitals, assisted living and adult home facilities, geriatric care managers, financial planners, social workers, discharge planners, physicians, mental health providers, community groups, senior centers, elder care attorneys, guardianship agencies and others to establish and expand relationships and encourage appropriate referrals.
• Arranges presentations on Kadima for staff meetings in hospitals, nursing homes, doctor’s offices, and private companies.
• Acts as liaison for any clinical questions or documents requested by the facility’s Nursing staff on potential referrals or residents currently hospitalized and assist with the screening of potential residents.
• Works with Kadima development team to best represent the continuum of care provided by the network. Will make referrals to other providers in Kadima network as appropriate.
• Communicates with potential residents and families informing and assisting them with the admission process.
• Works on obtaining HMO and other third-party authorizations for accepted referrals once placement is confirmed.
• Will assist in the planning and hosting of special events held at Kadima.
• Accounts for daily usage of time and identifies contacts made as well as referrals obtained through an End of Day email log.
• Provide direct supervisor(s) with anticipated weekly schedule for community outreach and referral activity.
• Performs other duties as assigned
